This form can be filled out on your computer or printed and filled out by hand. 

 

To fill out on your computer:

• Use "Save as" to save the file to your computer. Please give the file a new name with your name in the title. 

• Fill out the form.

• Save the form again when you have finished filling it out.

• Email the form to kelly@kaizenhealingarts.com or print it and bring it to your appointment
